Questionnaire for Vitalité Health Network patients - Virtual health care service delivery

Different ways of delivering health care services are emerging during the pandemic. Around the world, efforts are underway to deliver health care services virtually, that is outside hospitals, clinics or even doctors’ offices. An example of this would be a patient having an appointment from their home with their doctor, nurse practitioner or health professional using technology (telephone, computer or videoconferencing).

Vitalité Health Network wants to know what you think about this topic and to get a good understanding of your needs. This is why we are inviting you to complete this questionnaire until August 10^th.
