Northwest: improving access to services for vulnerable people
Two Moving Forward Together community clinics, held in the Edmundston and Grand Falls regions, provided a range of essential services to people experiencing homelessness or living in highly vulnerable situations.
Participants were able to access health and social services in a single location, and receive support in government processes.
Coordinated jointly by Service Canada, the Northwest Regional Service Commission and Vitalité Health Network Public Health, with the support of numerous partners, the initiative helps reduce administrative barriers, strengthen access to public services and promote greater social inclusion.
Several Network teams played a key role, including public health nurses and dietitians, the addiction and mental health services team and primary health care nurse practitioners, to provide immunization, counseling, psychosocial support, guidance and clinical assessments.
