Moving toward more accessible, better coordinated team-based care
The collaborative model of local family health teams continues to gain ground in the communities served by the Network. Two new teams have been added, one in Moncton and one in Dieppe, bringing the total number of teams established to 34.
The Moncton team is made up of three physicians, while the Dieppe team consists of four physicians. The first cares for 1,779 patients who now also have access to the services of a social worker. The second team will strengthen primary health care in the Moncton area.
Since local family health teams were established:
- 45,959 additional patients have been taken on;
- 26 of the 34 active teams offer access within 48 hours for minor emergencies, and
- 95% of the population served by the Network now has access to a primary health care provider.
Over the last quarter, a new local family health team was also set up in Beaurivage, Kent County. Three physicians make up this team that already cares for approximately 3,250 patients.
The team has also expanded its services with the addition of licensed practical nurses, a social work graduate and administrative support staff. As its capacity grows, it should be able to take on 1,000 more patients over the next 12 to 18 months, contributing to the stability of services in the community.
In Memramcook, the Clinique des Trois Moulins team will soon operate from a newly renovated point of service. The clinic now has twice as many examination rooms, creating favourable conditions to expand the team, increase patient capacity and enhance services offered to the public.
This team, made up of three physicians and a nurse, is now supported by a social worker. A licensed practical nurse position has also been added. A new phlebotomy service will also bring more care directly into the community.
