Improving access to primary health care
The Network's strategy for improving access to primary health care is based on the creation of local family health teams.
This innovative model is based on patients being cared for by a multidisciplinary team, rather than by a single doctor. This approach makes it possible to delegate certain tasks and set up a rapid treatment system for minor emergencies.
The sharing of electronic medical records facilitates collaboration between different health care professionals and ensures continuous follow-up of patients throughout their care journey.
The deployment of local teams contributes to the achievement of three main objectives:
- Increasing the number of people assigned to a collaborative care team;
- Offering routine appointments within five days for non-urgent consultations;
- Offering appointments within 48 hours for minor emergencies, to guarantee rapid care for these situations.
