Transformational leadership: What it is… and what it is not
In a constantly evolving environment where change is accelerating, an organization like our health network cannot operate as if it were in a period of stability. It must adopt transformational leadership—an adaptive approach that is attentive to current realities and responsive to emerging challenges.
Transformational leadership starts with a mindset focused on taking responsibility, pursuing growth, and considering the impact of our actions. It means anticipating challenges rather than simply reacting to them, and bringing structure to complexity instead of being constrained by it. This approach views the organization as a system that must be aligned, mobilized, and continually improved.
At the heart of this approach is a sense of ownership.
A person who practices transformational leadership acts as if they own the organization—even without formal authority. They protect resources as if they were their own and make decisions with the long term in mind, understanding that every choice affects the organization’s overall direction.
However, mindset and intention alone are not enough.
Transformations rarely fail due to a lack of strategy; they more often fail because of poor execution.
Strategic execution depends on five key elements:
- Clarity of priorities: knowing what truly matters and accepting that not everything can be done.
- Organizational alignment: linking objectives to behaviours, processes, and performance indicators.
- Rigorous accountability: measuring results, making adjustments, and acting quickly when needed.
- Clear boundaries: defining what is not tolerated, both in performance and behaviour.
- Determined follow-through: removing obstacles that slow down action.
Transformational leadership fosters a culture where expectations are clear, where a sense of ownership is expected, and where that ownership is translated into concrete, measurable actions that align with the strategy. Above all, it keeps sight of the people for whom we do this work: patients, their families, and our communities.
Although sometimes portrayed as grand or heroic, transformational leadership is grounded in the everyday. It shows up in daily behaviours, disciplined execution, and the willingness to solve problems rather than avoid them. It rarely looks like the inspirational image often associated with leadership. It is demanding, consistent, and deeply rooted in action.
Transformational leadership is something we can all practise. Every decision we make, every action we take, and every habit we build can contribute to transforming our health system.
