This exercise and education program is for people with heart disease. It helps people recover and get their strength back and reduces their risk of a relapse.
The program:
- Improves physical fitness and the ability to do everyday tasks;
- Improves quality of life and self-confidence;
- Reduces the risk of a relapse by reducing risk factors;
- Improves knowledge of risk factors;
- Prepares people to go back to work.
The program is free. It lasts 12 weeks. It can be adjusted to a patient’s medical condition, needs, and tastes. It includes:
- Consultation and follow-up services;
- Assessment of cardiovascular health at the beginning and end of the program;
- Advice on diet;
- Advice on cholesterol control, high blood pressure, weight, diabetes, and quitting smoking;
- A personalized exercise program that is effective, safe, and tailored to a patient’s health, age, and fitness level;
- Exercise sessions supervised by professionals or a personalized home exercise program;
- Information on depression and coping with other problems related to heart disease.
Our multidisciplinary team includes:
- Physicians;
- Nurses;
- Dietitians;
- Kinesiologists or physiotherapists;
- Psychologists.
