The vision of the diabetes clinics is to improve knowledge about diabetes and encourage self-management among people living with diabetes.
Diabetes educators have extensive knowledge and expertise in diabetes care. They can also support healthcare providers by offering up-to-date recommendations to help optimize diabetes treatment. Insulin adjustments may also be made according to the patient's condition and needs.
The clinics also provide:
- Follow-up care for patients with diabetes of all ages.
- Follow-up care for patients with diabetes who use insulin pumps or continuous glucose monitoring systems.
- Follow-up care for patients with gestational diabetes during pregnancy.
The clinics offer diabetes education:
On a voluntary basis and free-of-charge.
Patients learn how to:
- Develop and maintain healthy lifestyle habits.
- Adopt healthy eating habits.
- Understand diabetes and effectively manage their blood glucose levels.
- Learn to self-manage their treatment.
- Improve their independence and self-management skills.
- Take an active role in managing and controlling their diabetes.
Who is eligible for services at the clinics?
Anyone who has been diagnosed with diabetes.
How to request services?
You can contact the clinic directly to request services (self-referral). A physician's referral is not required.
Your physician or nurse practitioner may also complete referral form RC-316 and forward it to the appropriate clinic.
