The new online self-booking service now lets you select your preferred date and time for all general X-rays.
The service is simple, secure and available at all times. Our aim is to provide you with faster, more flexible access that’s better tailored to your needs.
For the time being, this self-booking service is only available in the Northwest region. However, patients wishing to travel to this region can access the service.

Who can use this service?
You can use this online self-booking service if you have an X-ray order (requisition) from a physician or nurse practitioner.
How to use this service
Once you’ve received your requisition, simply log using a computer, tablet or phone and enter the required information.
How do I book an appointment online?
Book an appointment in four simple steps:
- An order (requisition) from your medical practitioner is required for this exam.
- Access the online platform from your computer, phone or tablet.
- Select the preferred date and time for your appointment.
- Complete the application form: you'll need to enter your personal information.
Yes, you must have an order from your medical practitioner, otherwise you won't be able to book an appointment online.
A Vitalité Health Network general radiography requisition form must be completed, including the patient's personal information, relevant clinical information, the type of exam requested and the prescriber's signature.
Yes, some appointment requests will be accepted, such as Horizon Health Network's general radiography requisition form. For more information, contact your appointment centre.
No, you may choose the facility that works best for you.
At the moment, we only offer appointments for general X-rays in the Northwest Zone. Other services will be added in the future.
Check your confirmation email for the "Reschedule" link if you need to change your appointment. If you need help, contact your appointment centre.
Yes. In this case, be sure to include the patient's first and last name and telephone number, or provide the number of a person authorized to discuss the patient's health, should the appointment centre need to contact the patient.
