Roles of the Health Records Department
- Put together a complete and accurate medical record that reflects the course of medical activities to ensure continuity of care
- Verify, index, file and retrieve medical records for any patient who goes to one of the Network’s hospitals
- Ensure the availability and retention of the complete medical record for each patient treated in the Network’s hospitals
- Promote confidentiality
- Protect the legal rights of the patient, hospital and medical staff by maintaining accurate records
The medical record is used in the planning and continuity of patient care. It serves as a means of communication between the physician and all other professionals who participate in the patient’s treatment.
See below to obtain a copy of your medical record or request its transfer.
Medical records held in a private practice:
Contact your physician directly.
In some cases, when a physician leaves their practice, the physician may use an outside supplier to manage their files. Follow the instructions provided by your physician to obtain a copy of your record. This practice is in accordance with guidelines of the College of Physicians and Surgeons of New Brunswick and the New Brunswick Medical Society.
Medical records at Vitalité Health Network
To obtain a copy of your medical record or have it transferred from a Vitalité Health Network point of service:
- Complete form RC-12.
- Submit the form by email, fax, mail, or in person to the Health Records Department at the hospital
- If you have been hospitalized in more than one location, indicate Vitalité Health Network on your request
- It may take up to 30 days
- Fees may apply
